We are determined to prevent chronic conditions from taking away each individual’s quality of life.
The 2016 Jersey Health profile revealed that, in total, around 27,000 registered patients had at least one chronic condition at year-end 2015, equivalent to 24 per cent of the patient population, or one in four patients.
